Please contact the School Nurse if your child requires medication or has a chronic health condition that could affect their time here at school.  We will be happy to discuss any questions or concerns you might have.

Hickory Public Schools personnel or School Health Nurse will administer prescription and over-the-counter medication to students during school hours if required by the student’s physician.  It is the parent or guardian’s responsibility to obtain and sign a “Parent/Guardian Request for Medication” form and have it completed by the physician for all medication to be given at school.  All medications, prescription and non-prescription, must be sent to school in the original container from the pharmacy.  Prescription medication must have a current label with the child’s name, doctor’s name, name of medication, unit dosage to be given, and how it is to be given.  All medications will be kept in a locked area.  Students may not carry medication with them unless they have a specific order to do so by their physician (I.e. inhalers or epi pens).  Records of all medications administered are kept on file in the school health room.
